PRESCRIBED EXERCISE TO ASSIST YOU
Chronic disease management involves prescribing exercise for clients with ongoing medical conditions.
These conditions can be managed with a specified exercise program. Clients may be eligible for Medicare-funded sessions under a GP management plan and can also claim sessions through their health fund.
We use evidence-based practice to prescribe exercise for a number of chronic conditions including, but not limited to, those listed below:
- Osteoporosis
- Diabetes (type I and II)
- Cardiovascular disease
- Hypertension

Individuals with a diagnosed chronic health condition, such as diabetes, cardiovascular disease, arthritis, respiratory conditions, and more, are eligible for chronic disease management services. A referral from your GP is often required.
During your first session, you will undergo a comprehensive health and fitness assessment. Your exercise physiologist will discuss your medical history, current health status, and specific goals to create a personalized exercise plan.
The frequency of sessions will depend on your individual health condition and goals. Your exercise physiologist will recommend a suitable schedule to help you manage your condition effectively.
Yes, our exercise physiologists can design programs that you can perform at home, at your local gym, or within our private training facility in O’Connor. The exercises will be tailored to your environment and resources.
